Volunteer Forms
Our Religious Education Program is dependent upon generous volunteers who make a difference in the lives of the children of our parish by sharing their time, talents and faith. Please consider joining our staff as a Catechist/teacher, Aide, Substitute, Hall Monitor or Office Support. Thank you!
Confirmation Forms
Confirmation canditates prepare for Christian life by fullfilling service hours. We have provided a guideline to help you choose projects that are rooted in the Spiritual and Corporal Works of Mercy. The Service Report form is used to record and submit hours to our office.
7th Grade Forms
Confirmation canditates complete an application form during their second year of preparation for the sacrament. A guideline for selecting a confirmation name and preparing a report on the choosen saint is provided.
8th Grade Forms
Confirmation canditates select their sponsor and provide a letter of intent to the pastor during their final year of preparation for the sacrament.
